Our Community nurses provide comprehensive wound care encompassing assessment, cleaning, dressing selection, wound protection, pain management, monitoring, education, and coordination of care. They conduct thorough assessments to determine the type and severity of wounds using appropriate techniques and materials to promote healing and prevent infection and monitor progress while adjusting treatment plans as necessary. They will also educate participants, and their caregivers on proper wound care techniques, provide support, and collaborate with other healthcare professionals to ensure holistic and coordinated care.
Our nurses are trained in diabetes management, they offer personalised support for maintaining healthy blood sugar levels and promoting optimal self-care practices. They provide comprehensive support, including education on diabetes management, medication administration, blood glucose monitoring, dietary advice, exercise guidance, and foot care. Our nurses conduct regular assessments to monitor blood sugar levels, assess for complications such as diabetic foot ulcers, and provide preventive measures to minimize risks. They collaborate closely with participants to develop personalized care plans tailored to the individual’s needs and abilities. Through their expertise, advocacy, and ongoing support, community nurses empower individuals to effectively manage their diabetes and enhance their overall quality of life within the framework of the National Disability Insurance Scheme (NDIS).
